I've been working my way through a large collection of cigar boxes recently. This paper covered wooden cigar box seemed perfect for a couple of radio tube bulbs, so that's what I did. It has modern bakelite style sockets and a chrome toggle switch (which makes a very satisfying "click-clack" every time you turn it on and off.) It has a 6 foot long cord so you shouldn't have any trouble reaching an outlet. Comes with 2 incandescent radio style bulbs, but will fit any standard sized bulb.
